What Is Testosterone?

Therefore, testosterone treatment should be considered only after appropriate evaluation.

TRT and Erectile Dysfunction

Some men with confirmed testosterone deficiency may also experience sexual-function problems.

However, TRT should not be marketed as a universal treatment for Erectile Dysfunction. ED can have vascular, metabolic, psychological, medication-related and other causes.

If a patient has ED together with symptoms suggesting testosterone deficiency, hormone evaluation may form part of the overall assessment.

TRT and Fertility

An important consideration before starting TRT is future fertility.

Testosterone therapy can suppress the body’s own reproductive hormone signalling and sperm production. The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term.

Men who are trying to conceive should therefore discuss their fertility plans with the doctor before considering testosterone treatment.

Is TRT Safe for Everyone?

No.

TRT is a prescription medical treatment and is not suitable for everyone. Certain medical conditions and circumstances require avoiding or carefully evaluating testosterone therapy.

Potential issues requiring medical consideration include elevated haematocrit, certain prostate-related concerns, untreated severe sleep apnoea, significant cardiovascular conditions and fertility plans.

Recent FDA information also notes that testosterone products can increase blood pressure, so appropriate medical supervision and monitoring are important.

Monitoring During TRT

If testosterone treatment is prescribed, follow-up is important.

Monitoring may involve assessing:

  • Haematocrit and blood counts
  • Blood pressure

  • Possible adverse effects
  • Ongoing need for treatment

The Endocrine Society recommends evaluating men after treatment begins to assess response, adverse effects and treatment adherence.

TRT Is Not a General Anti-Aging Treatment

Feeling tired, having lower libido or noticing changes with age does not automatically mean that testosterone replacement is needed.

The appropriate approach is to determine whether genuine testosterone deficiency is present and whether there is a reversible underlying cause.

For example, the Endocrine Society’s 2026 statement notes that obesity and certain medications can contribute to low testosterone and that reversible causes should be addressed where appropriate.
